Financial information

Dr. Martini is contracted with the following Healthcare Insurance Companies:

Cigna.

Dr. Martini is NOT contracted with Medicare, Medicaid, Tricare, L&I or DSHS.

If you have healthcare coverage through other carriers we will provide you with a bill to submit directly to your insurance company for reimbursement.

We do not bill companies on behalf of patients if we are not contracted with them.

Patients are expected to pay their portion at the time of service as we no longer send bills except for rare situations.

The practice accepts cash, debit/credit card payments for co-pays and out of pocket costs. We only accept personal checks from established patients.

Please have your insurance information ready so we can verify your benefits and advise you of your out-of-pocket charge. If we cannot verify the benefits you will be required to make full payment at time of service.

CANCELLATION POLICY:

• TO AVOID CANCELLATION CHARGES PLEASE MAKE SURE TO CANCEL YOUR APPOINTMENT 48 HOURS IN ADVANCE.

• CANCELLATION CHARGES ARE:          $100 FOR 30 MIN OR LESS APPOINTMENTS
